Requirements
APPLICATION REQUIREMENTS
• Principal card holder - Minimum age is 21 years old and maximum age is 60 years old
• Supplementary card holder - 18 years and above
• Platinum - Minimum income is RM 96,000 per annum
• Gold Card - Minimum income is RM 36,000 per annum
• Classic - Minimum income is RM 18,000 per annum
REQUIRED DOCUMENTS:-
– Copy of NRIC (both sides) or passport (also for supplementary cardholder, if required)
– Latest two months’ salary slip
– Latest income tax return (form J/ EA)
– Copy of NRIC (both sides) or passport (also for supplementary cardholder, if required)
– Copy of Business Registration Certificate or Form 9,24 and 49.
– Latest J Form/EA (Income Tax Return)
– Three months’ bank statements

– Copy of credit/charge card
– Copy of last three months’ credit/charge card statements
