| Eligibility (Salary Earner) |

- Malaysian citizen.
- Fixed income earner with a minimum gross income of RM2,000 per month.
- Minimum tenure in service:
- Permanent Staff : 6 months
- Contract Staff : 2 years (Government only)
- Maximum total salary deduction of 75%

For salary earner applicant:-
- Photocopy of identity
- Latest payslip
- Latest Letter of confirmation from employer
- Certificate of registration with Malaysian Medical Council or Malaysian Dental Council
- Latest Annual income statement (EA/EC/B Form) OR latest 3 months Bank Statements of salary credited account OR any other supporting document such as EPF statement, if required.
For Sole Proprietorship or Partnership:-
- Photocopy of identity card
- Business Registration.
- Last 6 months current account statement.
- Latest Income tax form (J/BE Form) with payment receipt
- Last 2 years financial statement
- Certificate of registration with Malaysian Medical Council or Malaysian Dental Council.
